Required forms for initial application:

Personal information

1. Completed application (see below)
2. Three (3) letters of reference
3. Personal goal statement
   • How will your current course of study enable you to reach your goals?

Education

1. High school diploma/GED certificate, if this is your 1st Semester
2. Latest college transcript
   • Letter of acceptance/admission if college transcript not available
3. Class schedule
   • You may not yet have this on interview date but must be submitted before award is presented.
4. Financial
   1. Pell Grant acceptance or denial letter
   2. Proof of residence (Example: Utility bill with your name, and copy of lease/rent)
